What is an Asthma Symptom Diary?

An asthma diary helps you keep track of how well you are managing your asthma. You can track what triggers your asthma attacks, symptoms, medications you used and how well they worked.

Why Use an Asthma Symptom Diary?

An asthma symptom diary can help you and your healthcare provider manage your or your child’s asthma, which can help guide evaluation, management or treatment decisions. However, please note that this table is not a substitute for medical advice. If your symptoms are severe or worsening, it's important to seek medical attention.

How to Use an Asthma Symptom Diary?

Once you print out the template, here’s how to track symptoms and medications.


Make sure you consistently track these symptoms every day. A tick mark if the symptom was present and a 0 or a cross if it was not. Please remember to count weeks - Every weekly block has a field where you should count the week (week 1, week 2, week 3, etc)


Fill in your medication on the left. Use the fields corresponding to each day of the week to note the exact time when you took each medication. If you're sharing this with a healthcare provider, be sure to check with them to see if there's anything else they want you to track.
